404 error on the App Part

Hi,

I have created a SharePoint Hoster App and uploaded it to the App catalog. When I click on my App on Site Conetnt Page its going to app page and working perfectly but  when I add the App part from the ribbon on my page, the app part shows 404 error.

Can anyone help me on how to resolve it?
